About the Position
Provides direct patient care and services to health plan members in the area of Psychiatric Social Work, including assessment, diagnosis, treatment, and crisis intervention.
Role
The LCSW provides focused care on discharge planning for patients and families within inpatient and outpatient medical settings as part of the health care team. Works with individuals, families, groups to promote optimum mental, emotional, and social health. May help individuals and families with chronic illnesses, substance abuse, unwanted pregnancies, or unemployment; crisis intervention. The LCSW may also participate in department medical rounds, develop and implement treatment plans, and provide referrals to community resources.
